Physical Therapist - $60/hr - Glendale, AZ
Position Overview:
We are seeking a Physical Therapist to provide evaluations and therapy services for students in grades K-8. This role includes performing assessments, implementing therapy plans, and documenting services in accordance with physician orders and individualized plans of care.
Assignment Details:
+ Location: Glendale, AZ
+ Start Date: December 18, 2025
+ End Date: End of 2025-2026 School Year
+ Schedule: Monday-Friday, 40 hours/week
+ Duration: 36 weeks
+ Pay Rate: $60/hour
+ Modality: In-person
+ Caseload: Split between 2-3 campuses (all within 15-20 minutes of district office)
Responsibilities:
+ Conduct evaluations and develop individualized therapy plans
+ Provide physical therapy services in accordance with physician orders
+ Document therapy sessions and maintain compliance with district and state guidelines
+ Collaborate with teachers and multidisciplinary teams
Requirements:
+ Current Arizona Physical Therapist license
+ Graduate of an accredited Physical Therapy program
+ One year of prior professional PT experience preferred
+ Must meet all federal, state, and local requirements
+ Current CPR, TB questionnaire, and health certificate if applicable
+ Must be at least 18 years of age
