Position Overview

The Director of Nursing (DON) is a senior clinical leader responsible for overseeing nursing services, clinical quality management, compliance, and continuous quality improvement across all agency programs. This role ensures the delivery of high-quality, compliant, and person-centered care in accordance with federal, state, and contractual requirements.

Working closely with executive leadership, case management, and operations teams, the Director of Nursing leads audit readiness, clinical performance monitoring, corrective action planning, and workforce development initiatives. This is a highly metrics-driven role , requiring strong leadership in data analysis, regulatory compliance, and operational execution within home and community-based care environments.

Key Responsibilities Clinical Leadership & Strategy
  • Provide executive-level insight to the CEO on trends in clinical services, case management, intake, HR, service delivery performance, documentation quality, and staff and client satisfaction.
  • Collaborate with senior leadership to establish and maintain clinical quality standards, benchmarks, and performance metrics.
  • Align nursing policies, procedures, and clinical workflows with best practices and evolving regulatory requirements.
  • Participate in strategic planning and support the development of new clinical programs and service models.

Quality Assurance, Audit & Compliance
  • Lead the Agency’s Clinical Audit and Quality Improvement Program in coordination with regulators and contractors.
  • Oversee internal and external audits (OMIG, DOH, HRA/MCOs, and third-party vendors), including preparation, execution, remediation, and follow-up.
  • Develop and monitor corrective action plans (CAPs), root cause analyses, and remediation tracking to resolution.
  • Maintain regulatory reporting logs and ensure timely submission of all required clinical and compliance reports.
  • Provide monthly compliance and audit reports to the CEO and DBO; present quarterly reports to the CQI and Corporate Compliance Committees.

Continuous Quality Improvement (CQI)
  • Lead CQI initiatives focused on Clinical documentation quality and timeliness, Nursing visit compliance, EVV implementation and oversight, Overtime monitoring and control, Incident and complaint management, and Internal claims audit remediation.
  • Conduct regular clinical chart reviews, focusing on high-risk clients and those with prior incidents or complaints.
  • Monitor trends in incidents, complaints, and gaps in care delivery, implementing corrective interventions as needed.
  • Oversee client and home care worker satisfaction surveys and translate findings into measurable improvement actions.

Staff Development & Oversight
  • Supervise and mentor nursing leadership and the Associate Director of Quality & Compliance.
  • Identify, design, and implement clinical training initiatives to strengthen staff competency, compliance, and service quality.
  • Support department leaders with coaching on documentation standards, audit readiness, and quality oversight.
  • Ensure consistent implementation of standardized quality, audit, and compliance programs across all agency services.

Reporting & Performance Management
  • Prepare and present clinical quality dashboards, audit summaries, and trend analyses for leadership and governance.
  • Host bi-weekly interdepartmental quality improvement meetings.
  • Track and report progress against defined Key Performance Indicators (KPIs).
  • Maintain logs of quality initiatives, CAPs, training activities, and improvement projects.

Qualifications
  • Licensed Registered Nurse (RN) – Required
  • Bachelor’s degree in Nursing, Human Services, Social Work, Healthcare Management, or related field (Master’s preferred)
  • Minimum 8 years of professional experience
  • At least 3 years of experience in home care or community-based healthcare
  • At least 3 years of supervisory or leadership experience
  • Strong analytical, problem-solving, and organizational skills
  • Proficiency with Microsoft Office (Word, Excel, Outlook, PowerPoint)
  • Ability to multitask, manage time effectively, and work independently
  • Bilingual capability is a plus
