Job Description: We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our healthcare team in Kalamazoo, MI and Chicago, IL. The ideal candidate will be responsible for providing comprehensive, patient-centered nursing care in the comfort of patients’ homes, ensuring quality outcomes and continuity of care.
Responsibilities:- Deliver one-to-one skilled nursing care to patients in their homes in accordance with physician orders and the individualized plan of care.- Conduct comprehensive patient assessments, including physical, emotional, and psychosocial needs.- Develop, implement, and revise care plans to meet patient goals and clinical outcomes.- Administer medications, perform wound care, monitor vital signs, and provide other clinical interventions as needed.- Educate patients and families on disease management, medication adherence, and self-care techniques.- Collaborate with interdisciplinary healthcare teams, including physicians, therapists, and case managers, to coordinate care.- Document all patient care activities accurately and promptly in compliance with state and federal regulations.- Monitor patient progress and communicate changes in condition to the care team and physician.- Ensure compliance with infection control standards and agency policies.- Provide emotional support to patients and families while promoting independence and quality of life.
Qualifications:- Active and unencumbered Registered Nurse (RN) license in the State of Michigan (required).- Minimum of 2 years of clinical nursing experience, preferably in home health.- Strong knowledge of nursing principles, practices, and standards of care.- Excellent clinical assessment and critical-thinking skills.- Proficiency with electronic medical records (EMR) systems.- Strong communication, organizational, and interpersonal skills.-
Valid driver’s license, reliable transportation, and proof of auto insurance.
